Would you rather swallow one small tablet every morning and stop planning, or swallow a larger tablet when a night is actually on the calendar? That is the whole fork. Daily ED dosing on Cialis starts at 2.5 mg once daily and may rise to 5 mg. As-needed dosing starts at 10 mg and may rise to 20 mg or fall to 5 mg. You do not run both schedules at once.
The molecule does not change personality when the milligram shrinks. What changes is whether plasma sits at a low steady level or spikes around a date. Steady-state concentrations arrive within 5 days of once-daily dosing, about 1.6 times a single-dose exposure. Skip days and you are not on the daily product the label described.
What happens if the daily 5 mg is forgotten half the week?
Missed daily doses undo the steady-state story. The label's continuous plasma line assumes the tablet is taken. Five days to steady state is not a license to 'catch up' with two tablets. There is no extra Saturday 10 mg stacked on a 5 mg habit unless a prescriber redesigned the order.
If you already know you will forget pills that are not tied to a visible routine, PRN 10-20 mg is the more honest order even if you wish you were a daily person. Wishful adherence is how people end up with a half-used 5 mg bottle and a leftover 20 mg strip and no idea which level they are on.
Travel days break routines. If the daily tablet lives next to a toothbrush, it usually survives a trip. If it lives in a drawer you only open on date night, you were never on daily dosing. Say that out loud before you pay for thirty tablets you will leave at home.
Who has no schedule to choose?
Nitrate users have no schedule. Regular or intermittent organic nitrates are contraindicated with Cialis. If nitroglycerin is later judged necessary in a life-threatening setting, at least 48 hours must pass after the last tadalafil dose, and even then only under close monitoring. Daily 5 mg does not make that safer. It keeps drug on board every day.
Strong CYP3A4 inhibitors (ritonavir, ketoconazole, itraconazole) cut the as-needed max to 10 mg no more than once every 72 hours, and cut daily use to 2.5 mg. Severe hepatic impairment: Cialis is not recommended. Those are stops, not bargaining chips. Riociguat sits in the same forbidden cGMP stack.
What is the 36-hour line, if not a 36-hour erection?
Thirty-six hours is a coverage window versus placebo, not a continuous erection. Tadalafil helps only when there is sexual stimulation. Hour thirty looks like hour two in that respect: no arousal, no effect.
The mean terminal half-life is 17.5 hours. That long tail is why daily dosing is even plausible, and why a second PRN tablet the same day is the wrong idea. Maximum frequency is once per day for most people. Kidney cuts are tighter: at creatinine clearance 30-50 mL/min the as-needed start is 5 mg, and 10 mg is at most once every 48 hours. Below 30 mL/min or on dialysis, as-needed max is 5 mg every 72 hours, and daily use is not recommended.
An erection that lasts more than 4 hours is priapism territory and needs same-day care. That clock is not the 36-hour marketing line. Do not wait it out because a forum said Cialis 'lasts two days'.
What are you paying for: thirty mornings or a handful of windows?
A month of 5 mg is a subscription to readiness. A box of 10-20 mg is a stack of weekends. I will not invent a US cash price. Count tablets against occasions. If you would have taken PRN eight times, daily is not 'extra medicine' in swallow count. If you would have taken PRN twice, daily is a lot of extra medicine.
Back or muscle ache that starts 12-24 hours after a dose and fades within 48 hours is a known tadalafil pattern. It shows up on both schedules. It is not proof the daily dose is 'too strong' by itself, and it is not a reason to add a second PRN tablet. If it wrecks the next day, talk about a lower milligram or a different column, not a silent extra 20 mg.
Do night-time trips to the toilet vote for daily?
Prostate symptoms vote for the 5 mg daily path when the diagnosis is actually BPH and a clinician has said so. Cialis for BPH, or for ED plus BPH, is 5 mg at about the same time each day. The 10-20 mg PRN order is not the labelled LUTS regimen.
If an alpha-blocker is already on board for flow, adding tadalafil is a blood-pressure problem, not a two-for-one coupon. The Cialis label wants the alpha-blocker stopped at least one day before daily tadalafil is started for BPH. That is a clinic instruction, not a home experiment.
A weak stream also needs a work-up that is not this page. Infection, retention and cancer are not 'try 5 mg and see'. When the prostate is the reason for the order, you are not shopping a weekend 20 mg.
How many occasions would a PRN bottle actually cover?
Count the occasions in a typical month before you romanticize either order. If sex happens twice a week, a 10-20 mg tablet taken each time is eight to ten swallows, plus the leftover anxiety about whether tonight 'counts'. A 5 mg daily bottle is thirty swallows whether the month was busy or quiet.
If occasions are two a month, daily 5 mg is a lot of drug for a short list of nights. PRN 10-20 mg then looks like the honest order: one tablet, up to 36 hours of labelled coverage, no daily accumulation story. Guidelines that pooled daily versus PRN tadalafil did not find a clinically meaningful erectile-function gap that would force one schedule on every man. The split stays personal.
Spontaneity is the other count. Some couples hate the 'swallow, wait, perform' script. Daily dosing deletes that script after the first five days. Some people hate a daily reminder in the kitchen. PRN leaves the other twenty-five mornings empty. Neither preference is immature. Name it before you pay.
Can a 5 mg morning sit next to a 20 mg night?
Two labelled strengths in one bathroom is how people invent a third schedule the label never wrote. Daily 2.5-5 mg already keeps continuous plasma levels. Adding a 20 mg 'just in case' the same day is a second dose, not a clever hybrid. Once per day is the usual ceiling for as-needed Cialis. Daily use is already that once.
If the 5 mg habit is not enough after a fair stretch at steady state (give it those five days, then a couple of weeks), the labelled move is to stay on daily and go from 2.5 mg to 5 mg, or to abandon daily and use 10-20 mg PRN. It is not to swallow both. A prescriber who wants a different design has to write it. A leftover 20 mg from an old fill is not that design.
Alcohol is the other unofficial stack. Substantial intake plus tadalafil can drop standing pressure; the daily label tells physicians to keep that in mind because the drug is always there. A weekend 20 mg plus a long night of drinks is the same pathway on a taller peak. Neither schedule is a drinking tablet.
The one number to take to the visit
Walk in with one number: occasions per month, honestly counted. Add whether night-time voiding is already a diagnosed BPH problem. Add whether any nitrate, riociguat or strong CYP3A4 inhibitor is in the house. Those three facts pick the order more cleanly than a review-site trophy.
